   Once we receive your appointment request, our office will review it and send you the intake forms electronically. Completing these forms typically takes 5 to 15 minutes and helps us understand your needs before your visit.
    
3. Insurance Verification
   After you submit your forms, we will verify your insurance benefits or discuss self-pay options with you.
    
4. Appointment Confirmation
   Once your forms are complete and insurance has been verified, a team member will contact you by phone or email to confirm your appointment and address any questions you may have.
