Clear Path Psychiatry accepts referrals from primary care providers, therapists, emergency departments, and community health partners across Minnesota. Alexandro Vasquez, DNP, APRN, PMHNP-BC provides adult outpatient psychiatric evaluation and medication management, with most patients seen within one week of referral.

Our referral process is secure, HIPAA-compliant, and designed to minimize administrative burden on referring providers.

• Appointments are confirmed after intake documentation is completed
• With patient consent, we can coordinate care updates back to your practice

With appropriate patient consent, Clear Path Psychiatry collaborates with referring providers and may share treatment updates to support coordinated, continuous care.

We welcome communication with primary care providers, therapists, and other specialists who are involved in a patient's care. Coordination requests can be directed to our office by phone, fax, or email.

Clear Path Psychiatry is an outpatient psychiatric practice and does not provide:

• Emergency or crisis psychiatric services
• Inpatient psychiatric care or hospitalization
• Substance use disorder treatment programs
• Pediatric or adolescent psychiatric services (adults 18+ only)
• Psychological testing or neuropsychological evaluation

Patients requiring urgent psychiatric care should be directed to the nearest emergency department or crisis services. Call 911 or text 988.
