Referral Process
The process is designed to be straightforward for referring providers. A referral may be submitted before all supporting records are available.

Appropriate Referrals
Clear Path Psychiatry accepts referrals for adults experiencing a wide range of mental health conditions requiring outpatient psychiatric evaluation and medication management.
Generalized anxiety, panic disorder, social anxiety, chronic worry, and anxiety-related functional impairment requiring psychiatric evaluation and medication management.
Major depressive disorder, persistent depressive symptoms, burnout, low motivation, anhedonia, mood instability, and treatment-resistant depression.
Attention difficulties, executive dysfunction, concentration problems, hyperactivity, and suspected adult ADHD presentations not previously evaluated or currently unmanaged.
Mood cycling, suspected bipolar I or II disorder, hypomania, treatment-resistant depression with bipolar features, or need for diagnostic clarification and mood stabilization.
Trauma-related symptoms, hypervigilance, intrusive thoughts, avoidance, emotional dysregulation, and post-traumatic stress presentations in adult outpatients.
Complex psychiatric presentations requiring medication review, psychiatric consultation, second opinion, or comprehensive diagnostic evaluation when the clinical picture is unclear.

Collaboration

With patient consent, treatment recommendations and updates are coordinated back to the referring provider. Clear Path works collaboratively with PCPs, therapists, hospitals, case managers, and other healthcare professionals involved in patient care.
Medication records, prior psychiatric history, lab results, and other supporting documentation can be provided after referral submission. Additional information may be requested when clinically necessary.
