Referral Process

How to refer a patient to Clear Path.

The process is designed to be straightforward for referring providers. A referral may be submitted before all supporting records are available.

We contact the patient directly

Our office reaches out to the patient within one business day to begin intake and scheduling. You do not need to coordinate the appointment — we handle it from here.

Patient is evaluated within one week

Most referred patients complete their initial psychiatric evaluation within one week. In-person in Eagle Lake or via secure telepsychiatry — whichever is appropriate for the patient's location and preference.

Coordination back to your practice

With appropriate patient consent, Clear Path Psychiatry coordinates treatment updates and recommendations back to your practice. We welcome ongoing communication with referring providers throughout the course of treatment.

Appropriate Referrals

Conditions commonly referred for psychiatric evaluation.

Clear Path Psychiatry accepts referrals for adults experiencing a wide range of mental health conditions requiring outpatient psychiatric evaluation and medication management.

Anxiety Disorders

Generalized anxiety, panic disorder, social anxiety, chronic worry, and anxiety-related functional impairment requiring psychiatric evaluation and medication management.

Depression

Major depressive disorder, persistent depressive symptoms, burnout, low motivation, anhedonia, mood instability, and treatment-resistant depression.

Adult ADHD

Attention difficulties, executive dysfunction, concentration problems, hyperactivity, and suspected adult ADHD presentations not previously evaluated or currently unmanaged.

Bipolar Spectrum

Mood cycling, suspected bipolar I or II disorder, hypomania, treatment-resistant depression with bipolar features, or need for diagnostic clarification and mood stabilization.

PTSD & Trauma

Trauma-related symptoms, hypervigilance, intrusive thoughts, avoidance, emotional dysregulation, and post-traumatic stress presentations in adult outpatients.

Diagnostic Clarification

Complex psychiatric presentations requiring medication review, psychiatric consultation, second opinion, or comprehensive diagnostic evaluation when the clinical picture is unclear.

Collaboration

Coordinated care with referring providers.

Effective psychiatric care depends on communication, continuity, and shared clinical understanding. Clear Path Psychiatry emphasizes collaborative outpatient care across disciplines.

Communication & continuity

With patient consent, treatment recommendations and updates are coordinated back to the referring provider. Clear Path works collaboratively with PCPs, therapists, hospitals, case managers, and other healthcare professionals involved in patient care.

Supporting records may follow

Medication records, prior psychiatric history, lab results, and other supporting documentation can be provided after referral submission. Additional information may be requested when clinically necessary.

In-person and telepsychiatry options

Both in-person appointments in Eagle Lake and secure telepsychiatry throughout Minnesota are available — depending on patient preference, geographic location, and clinical appropriateness.

Family medicine physicians, internal medicine providers, and NPs/PAs managing patients with mental health conditions that benefit from specialized psychiatric evaluation.

Therapists & Counselors

Licensed therapists, psychologists, LPCs, and LCSWs seeking psychiatric medication management for patients currently in or beginning therapy.

Hospitals & EDs

Emergency departments and inpatient units discharging adult patients who need timely outpatient psychiatric follow-up within the community.

Social Workers & Case Managers

Community health workers, social workers, and case managers supporting adult clients who need psychiatric evaluation as part of a broader care plan.

Community Organizations

Community health centers, school-based programs for adult populations, employee assistance programs, and public health organizations across Minnesota.

Self-Referrals

Patients may also contact Clear Path Psychiatry directly. A referral is not required in most cases, although some insurance plans may require one. Most new patients are seen within one week.

Most patients referred to Clear Path Psychiatry are contacted within one business day and scheduled for an initial psychiatric evaluation within one week. Telepsychiatry appointments are available for patients anywhere in Minnesota, which can reduce scheduling delays significantly.

Will you send updates back to my practice? +

Yes, with appropriate patient consent. Clear Path Psychiatry coordinates treatment updates, medication changes, and clinical recommendations back to referring providers. To initiate care coordination, contact the clinic by phone or fax. Do not send protected health information by email.

Do you accept MA and Medicare patients? +

Clear Path Psychiatry accepts Medicare, Medical Assistance, and multiple commercial and regional plans. Network participation and individual benefits can vary, so patients should verify coverage with their insurer before the appointment. Self-pay options are also available. Can you see patients who need urgent outpatient follow-up after discharge? +

Yes. Clear Path Psychiatry can typically schedule post-discharge outpatient psychiatric follow-up within one week. For patients discharging from inpatient or ED settings who need prompt outpatient follow-up, call the clinic directly at (507) 654-0020 to discuss expedited scheduling options.

What if my patient is in crisis? +
