Secure intake paperwork is sent through the patient portal after scheduling. Completing forms before the appointment helps create a more focused and efficient first visit.

Attend Your Appointment

Meet with your provider in-person in Eagle Lake or through secure telepsychiatry. The first appointment is a comprehensive psychiatric evaluation focused on understanding your history, concerns, and goals for treatment.

Your First Visit What to expect at your initial evaluation Your first appointment is designed to feel thoughtful, collaborative, and unhurried. The goal is to understand the full context of what you are experiencing before discussing recommendations or treatment options.

Comprehensive history review

We review current symptoms, mental health history, medical conditions, medications, sleep patterns, stressors, and any prior treatment experiences to better understand the full clinical picture.

Collaborative discussion

Care decisions are made together. We discuss your goals, concerns, previous experiences, and what type of support would realistically feel helpful and sustainable for you.

Clear recommendations & next steps

You leave with a clear understanding of diagnostic impressions, treatment recommendations, follow-up expectations, and practical next steps — without feeling rushed or uncertain.

A note from our provider

The first appointment is centered on listening. Before discussing medications or treatment recommendations, the goal is to understand where you are, what you have already tried, and what kind of support would actually feel meaningful and sustainable.

You do not need to prepare a perfect explanation or have everything figured out beforehand. Showing up is enough.

Be Prepared What to bring to your first appointment Preparing ahead of time helps ensure your first appointment is focused, efficient, and clinically meaningful. Most patients already have everything they need.
For in-person visits
  • Your insurance card and photo ID
  • A current list of medications and dosages
  • Prior psychiatric or medical records relevant to your care
  • Any recent lab results, testing, or hospital discharge paperwork
  • Completed intake forms through the patient portal
For telepsychiatry visits
  • A smartphone, tablet, or computer with a working camera and microphone
  • Reliable internet or cellular connection
  • A quiet, private space for the duration of the appointment
  • Completed intake forms submitted before the visit
  • Your insurance card and current medication list available nearby

Coverage Insurance & Fees

Clear Path Psychiatry is in-network with many major insurance plans including Blue Cross Blue Shield, HealthPartners, UnitedHealthcare, Medicare, Medical Assistance (MA), and additional regional plans throughout Minnesota.

Self-pay appointments are also available for patients who prefer direct payment or do not currently have insurance coverage.
