A current list of medications and dosages
Prior psychiatric or medical records relevant to your care
Any recent lab results, testing, or hospital discharge paperwork
Completed intake forms through the patient portal

For telepsychiatry visits

A smartphone, tablet, or computer with a working camera and microphone
Reliable internet or cellular connection
A quiet, private space for the duration of the appointment
Completed intake forms submitted before the visit
Your insurance card and current medication list available nearby

Coverage Insurance & Fees

Clear Path Psychiatry is in-network with many major insurance plans including Blue Cross Blue Shield, HealthPartners, UnitedHealthcare, Medicare, Medical Assistance (MA), and additional regional plans throughout Minnesota.

Self-pay appointments are also available for patients who prefer direct payment or do not currently have insurance coverage.
